The Research Institute for Home Care and National Alliance for Care at Home Release Inaugural Medicaid Home Care Chartbook

(Alexandria, VA and Washington, DC) — The Research Institute for Home Care (the Institute) and the National Alliance for Care at Home (the Alliance) today released the 2026 Medicaid Home Care Chartbook, the first report of its kind providing a broad overview of the Medicaid Home and Community-Based Services (HCBS) user population, workforce, organizational trends, and economic contributions of Medicaid home care providers across the U.S.

This report is the first Medicaid Home Care Chartbook produced and released by the Institute, in partnership with the Alliance, building off the success and insights from the organization’s signature annual research products – the Home Health Chartbook and Hospice Chartbook released earlier this year. This research, compiled and charted by Health Management Associates (HMA), a leading national healthcare and human services consulting firm, summarizes and analyzes statistics on Medicaid HCBS from a range of government sources.

The 2026 Medicaid Home Care Chartbook includes a comprehensive snapshot of the national Medicaid HCBS landscape, including:

  • Medicaid enrollee and home care use data by state
  • Medicaid HCBS service types
  • Proportion of home care users who are dual eligible
  • Home care use across various Medicaid eligibility groups

The Chartbook also includes the first look at potential effects of H.R.1 on home care, the One Big Beautiful Bill Act, which made substantial changes to Medicaid eligibility and enrollment policies for individuals enrolled in the ACA expansion population. To better understand the dynamics of H.R.1 and its impact on home care, the chartbook examines:

  • Proportion of home care users enrolled via the ACA expansion
  • Projected enrollment changes of home care users due to H.R.1
  • Estimated number of individuals who receive home care and will be subject to mandatory cost-sharing requirements established by the legislation

On Thursday, April 30th, the Alliance and the Institute will host a webinar on the 2026 Medicaid home care Chartbook findings. This webinar will share critical data and information relevant to the care at home community. We invite Alliance members and the media to attend this important conversation.
