Innovations@home: Home Health Initiatives Reduce Avoidable Readmissions by Leveraging Innovation

The Alliance has compiled a series of case studies to illustrate how many home health care providers are decreasing avoidable hospital readmissions by using innovative programs and technology solutions to manage complex conditions such as heart failure.
