Home Health Literature Review

The Alliance recently commissioned a literature review focused on currently available literature on select interventions in home health and home-based care to improve understanding of evidence related to outcomes and best practices. Co-authored by researchers from Avalere Health, LLC, and Alliance Executive Director Teresa Lee, the literature review published in” “Home Health Care Management and Practice” “in October 2013 finds that home health agencies are in a unique position to improve patient outcomes and increase cost-effectiveness of care. Approaches considered include: care coordination and care transitions, telehealth, and data-driven quality of care monitoring.
